At ReproMed Ltd. ("ReproMed", "we", "us", "our"), we are committed to protecting the privacy of our patients and customers. ReproMed Ltd. has developed this Privacy Policy to describe the ways in which we will collect, use, disclose and otherwise manage personal information, including that collected on www.repromed.ca and www.repromedsiblingregistry.ca (the "Websites" or each a "Website").

COLLECTION AND USE OF PERSONAL INFORMATION

We collect personal information when you:

• Become a patient;
• Apply and become a donor;
• Sign up to use the Sibling Registry or similar services;
• Participate in interactive features on our Websites, such as discussion forums;
• Register a birth;
• Sign up to receive our newsletters;
• Purchase a product or service;
• Apply for a job;
• Communicate with us by e-mail or otherwise with a request or comment.

Clinic Services

When you become a patient we will collect personal information such as your name, address, telephone number, date of birth, health card number, physician information, physical and biological characteristics, personal and family medical history, relationship status, social history and occupation. We use this information to assess your individual health needs, advise you of your health care options and provide the treatment or other health care-related services you require or request. We may use your information in non-identified form for educational or research purposes. We will also use your personal information to contact you about your appointments and other matters.

When you apply to become a donor we will collect your name, address, telephone number, and date of birth. We may also collect information such as your physical and biological characteristics, personal and family medical history, relationship status, social history, occupation and criminal record. We use this information to assess your application and contact you regarding your ability to become a donor. We may collect additional information from you when you become a donor, such as your photo and updated personal and family medical and social history.

Website Services

Registration: In general, you do not have to register to visit our Websites. However, some of the services offered through our Website (such as the Sibling Registry) may require you to register and provide your name, address, telephone number, email address, and a password and nickname that you select. We may also collect certain information about your child, such as their name, date and city of birth, sex, and other supporting information or documents. We use this information to identify you or your child, connect you or your child with other registered users, and to provide and otherwise administer the services you request. We also use your email address to send you emails in connection with the services.

When you use some of our services, certain of your registration information will be available to other registered users. You can control whether some of your registration information (such as your name and email) will be visible to others using your account settings.

Forums: We may provide you with the opportunity to post comments or otherwise participate in discussion forums on our Websites. We may collect any comments, suggestions and other feedback you provide as postings to these forums. We may use your comments, suggestions or feedback to monitor and/or improve our service offerings and our Websites. Information that you provide in these comment areas are accessible to other users, and you should exercise caution when deciding to disclose any personal information about yourself or anyone else in postings on the Website.

Newsletters: From time to time we will provide you with an opportunity to sign-up for our email newsletter. If you wish to withdraw your consent to receive our email newsletter, please contact us as outlined below.

Birth Registration: You may choose to register your birth with us. To do so, we may collect your name, address, email address, phone number, and donor number, as well as the name, sex, date of birth and weight of your child. We use this information to keep track of births and assist us in making decisions about our service offerings. We may also use this information to send you a congratulatory gift.

Administrative Services

Payments: To purchase one of our services, we may collect your name, address, phone number, email address, applicable insurance information and your payment information. To purchase donor specimen we will also collect your physician and donor preferences. We use this information to process your payment and otherwise fulfill your order.

Careers: We may advertise employment opportunities on our Website or other media. In connection with a job application or related inquiry, you may provide us with certain personal information about yourself (such as that contained in a resume, cover letter, or similar employment-related materials). We use this information for the purpose of processing and responding to your application for current and future career opportunities.

Contacting Us: If you choose to e-mail or otherwise contact us with a request or comment, we may collect your name, address, telephone number, email address, physician information, and health card number. We use this information to contact you and address your request or comment. We also may use your comments to monitor and/or improve our service offerings and our Websites. We may retain this information to assist you in the future.

DISCLOSURE OF PERSONAL INFORMATION

We may disclose your personal information to physicians, nurses, technicians or other individuals who need that information to provide the applicable health care services. This may involve the disclosure of personal information to your physician and other treating health care providers. We may also disclose your personal information to the Ontario Health Insurance Plan ("OHIP"), private insurers or others, where applicable, to allow us to obtain payment for health care services we have provided.

We will not otherwise sell, rent, transfer or otherwise disclosure any of your personal information to third parties except as set out in this Privacy Policy.

Legal: We may provide your personal information in response to a search warrant or other legally valid enquiry or order, or to an investigative body in the case of a breach of an agreement or contravention of law, or as otherwise required or permitted by Canadian or other law. We may also disclose your personal information where necessary for the establishment, exercise or defence of legal claims. Service Providers: We may transfer (or otherwise make available) your personal information to third parties who provide services on our behalf. For example, we may use service providers to host our website or process payments. All of our service providers are required to maintain the confidentiality and security of your personal information and are prohibited from using your personal information for any purpose other than providing the services on our behalf or as otherwise required by applicable law. Your personal information may be maintained and processed by our third party service providers in the US or other jurisdictions. Purchase and Sale of a Business: In the event that we sell all or part of our business or make a sale or transfer of assets or are otherwise involved in a merger or business transfer, we may transfer your personal information to a third party as part of or in preparation for that transaction.

SECURITY & RETENTION OF PERSONAL INFORMATION

We have implemented reasonable administrative, technical and physical safeguards in an effort to protect against loss or unauthorized access, use, destruction, modification and disclosure of personal information in our custody and control.

To provide you with increased online security, access to certain personal information will be protected by a password that you select. We strongly recommend that you do not disclose your password to anyone. We will never ask you for your password in any unsolicited communication.

We have personal information retention processes designed to retain personal information for no longer than necessary for the purposes stated in this Privacy Policy or to otherwise meet legal requirements

ACCESS TO YOUR PERSONAL INFORMATION

You have the right to access, update and correct inaccuracies in your personal information in our custody and control, subject to certain exceptions prescribed by law. You may request access, updating or the correction of inaccuracies in your personal information by emailing or writing to us at the contact information set out below. We may request certain personal information for the purposes of verifying your identity when seeking access to or correction of your personal information.

INFORMATION ABOUT OUR WEBSITE

Website Information: In general, you do not have to register to use our Website. For example, you can obtain information on our programs and current research studies without providing us with any personal information. However, we collect the IP (Internet protocol) addresses of all visitors to our Website and other related information such as page requests, browser type, operating system, screen resolution and average time spent on our Website. We use this information to help us understand our Website activity, and to monitor and improve our Website.

Cookies: Our Website uses a technology called "cookies". A cookie is a tiny element of data that our Website can send to your browser, which may then be stored on your hard drive so we can recognize you when you return. For example, if you have registered for a service on our Website, these cookies may let us know who you are and assist us in accessing your account information (stored on our computers) in order to deliver personalized services. You may find that some parts of the Website will not function properly if you have refused cookies. CHANGES TO THE PRIVACY POLICY

This Privacy Policy may be updated periodically to reflect changes to our personal information practices. The revised Privacy Policy will be posted on the Website with the date on which it was last updated.
